The blue text in a GO-IEP audit message names the section that needs correcting. Below: what the common ones actually mean, how to fix each, and the official page to verify.
Present Level Areas of Need OR Accommodations – State Testing and Student Supports
Cause: Accommodations exist on the State Testing page and/or Student Supports page, but the need hasn't been indicated on the Present Levels page.
Open progress reports from the previous IEP
Cause: GO-IEP never auto-closes prior-IEP progress reports. A new IEP or amendment can't finalize while any remain open.

Amendment without a formal meeting: parent not marked present
Cause: For an agreed no-meeting amendment, GO-IEP still asks who attended. If the parent isn't marked present, the audit fires because services would start before the parent was notified of changes.
Initial Eligibility: audit warning at Audit/Finalize
Cause: The Initial Evaluation completion date hasn't been entered on the Timeline page.
ESY goal / ESY decision mismatch
Cause: A goal is marked as an ESY goal, but the IEP's ESY decision and services don't match. An ESY goal requires the ESY decision to be yes and an ESY service in the IEP.
